Catacombs - Killed all enemies, but no card given

I'm re-playing in Nightmare mode (v1.61), I've run through the Catacombs level twice, and both times it shows the number of monsters killed as less then the total that's supposed to be on the level, and this is after killing the end level zombie. The first time it showed 108 of 112, the second time it was 109 of 112. I see nothing else to kill, and the way the game locks you in areas until you've cleared them, I would assume that I took care of everything in order to get to the end zombie in the first place.

I tried installing the PowerMad script and using the 'PKPerfect' cheat, but that still doesn't correct the mismatched monster count and let me get the tarot card. Is this a bug in the game, or am I missing something?

The "pmmorethanperfect" command would have done the trick ;).

Yeah, some levels (like Catacombs or Military Base) actually have some kind of non-linear gameplay. Usually you are right: you enter a room, all doors are shut and only opened when you kill all enemies. That way you can't miss one of them. But in these particular levels, there are sideways you MAY take or not. Usually they are dead ends, but there is ammo, gold and -- enemies in them. If you replay the level, watch out for turnoffs on your way ;)

Yeah, you got to explore Catacombs - There's a small bunch of monsters in the right hand alcove after you cross the first bridge and kill the dagger throwing Vamp.

The second lot are after you cross the chasm with the megahealth, but before the section where you start at the top of the 'ramp'. There's a small passage somewhere off to your right where two zombie warriors are eager to make your acquaintance...
